During the initial assessment by the company RN a plan of care will be designed for you based on your specific needs and preferences.

You can opt to not receive this service even though you are on medication. Our goal is to assist in the creation of a home care medication schedule for proper medication management. We will provide you a caregiver who is also a Certified Medication Technician, and they will administer medication to the patient under the supervision of our RN. Our RN will:

• Examine your home care medication administration.
• Make a complete list of all prescription and over-the-counter medications that the patient is taking.
• Make updates on any medication changes.
• Educate the patient and/or caregiver on the correct dosage, frequency, and side effects.
• Examine the effectiveness and response to medications.
• Communicate with the patient’s doctor to ensure patient outcome.

Getting to a scheduled health care provider’s and/or social event appointment is key to one of our major concerns: our clients’ health and wellbeing. We assist you to set up appointments, keep reminders, and get you there.

Personal concierge services for the following based on your care plan.

• Grocery shopping
• Mail pick up/drop off
• Delivery pick up
• Pick up prescriptions

Activities of Daily Living are activities related to personal care. Some examples are bathing or showering, dressing, grooming, getting in and out of bed or a chair, walking, using the toilet, and eating. If one has difficulty performing an activity alone and without special equipment, or cannot perform the activity at all, because of health problems, the person is deemed to have a limitation in that activity. These are basic self-care activities that include, but are not limited to:

• Showering/bathing and grooming
• Maintaining personal hygiene
• Selecting appropriate clothing
• Dressing
• Feeding
• Mobility
• Toileting

Meal Planning:
It’s crucial that our clients feed on nourishing meals, so we provide assistance with Planning, Purchasing, Preparation, Serving, Feeding, and Cleaning up. The Instrumental Activities of Daily Living (IADLs) include more complex activities related to the ability to live independently in the community. These include but are not limited to:

• Scheduling appointments
• Managing personal events
• Shopping
• Preparing meals
• Housekeeping
• Transportation
• Taking medication(s)
• Laundry

Light housekeeping of your living space is one of the services we provide. It includes duties that promote a clean and healthy living environment. We want to give you peace of mind when it comes to finding and safely accessing everyday items in your home. During our initial free consultation, we will request you to state your preferences and priorities for cleaning and tidying up so we can include them in your overall plan of care.

Sometimes the house does get a little too quiet. Spending time on an activity one likes helps improve ones’ mental health and wellbeing. A caregiver will offer companionship while you tend to your hobbies.
